      Nurses fill in far more #Datix than Doctors, I believe because Doctors are less culturally obedient. We look at the forms and know nothing good can come of spending time filling those forms. Yet Doctors should have the insights to identify important risks & bad outcomes

      That #datix form is shockingly bad design for usability. No pre-population eg staff or pt. ID, ambiguous date etc. A #feralsystem, unlinked to any other NHS IT eg drug dix QA so important, needs integration with clinical systems on #openplatform Meanwhile use @CareOpinionScot?

      Indeed, and thank you for sharing on dropbox those screenshots of the UI. It's as if the designers don't realise that it's clinical time that is wasted, and their own product obstructs QA in healthcare. OTOH the myriad of incompatible silo systems ... Comment please @DatixUK ?

      That was what the Incident Reporting Software used to look like @westernsussex before #Datix Who are you? What happened? The reporter should not have to classify the incident nor do a whole lot of the work of an investigator

      Exactly, I have never understood http://that.Systems  I've used, clinicians are expected to judge how the incident poses a risk to the trust! And there is a weird sort of matrix about 'how serious' and 'how likely' that is v. rarely possible to guess at but guess one must.

      You'd think so, wouldn't you? And yet that's exactly what I and many others have had to do. Police seem to do all they can to avoid registering crimes these days. And best of luck to you if that crime happens to have been committed by a doctor. Your point stands though.

          In 1994 we submitted a file of evidence to Dyfed Powys Police & put the charges on a plate for them.           Anomalies with Robbie's computerised records. Firstly they weren't declared. When they were, only one of the 6+ sources of computerized records was revealed. Falsely claimed the PAS records were deleted when system changed. Then a highly suspect, inaccurate printout was revealed.pic.twitter.com/pW9NbT2gN1

          I can see from the Robbie's computer records as shown to you Will, that something sinister lies behind this Incomplete, inaccurate, riddled with anomalies to the point that Robbie isn't even recorded as dead on the last computer printout. Makes me so sad to see this. RIP Robbie x
