Not correct. I missed a case of Addison’s because I was angry and overworked. I can understand how vitally important notes (Do the synacthen tear) get overlooked in the chaos of paper or digital notes that exist in NHS especially across Hospital / GP divide.

And potential defamation when staff attack person's character rather than fess up, allow closure & learning will drive people to litigation to prove their point & protect their good name/ character.
-
They'd much prefer to gaslight rather than take responsibility. That's becoming painfully apparent all round.
-
Can't understand why, when it becomes so obvious, they try to keep it up. We no longer sit trying to sort out a problem like this with a letter (pen & paper). Our resources now mean it can backfire. Damages others trust in them. Yet they have been so slow to catch on.Embarrassing
-
My take is that they do it because they can. Who stops them? It's only recently that the means to counter their narrative has become available to the public. And they hate it.
-
True but thing is there R a few people I would not want to appear smiling in a photo with now because that would damage my reputation more than anything they have written about me if it appeared in public. If by any chance there was risk of that I would have to make a run for it

Indeed. Just make sure you choose a wider tree behind which to hide than Jeremy Hunt did!

Would be locked in the ladies with excuse of sudden bladder weakness. No fear of meeting these people but photo with them might give impression I am in league with them, or traitor to patients & too many have read evidence of what I know so I don’t have the excuse of being naïve.

As I always say; once you've seen the devil's tail, you can never unsee it.

Exactly! And if hospitals genuinely want to learn how to cut litigation, there's a list of small, simple measures that can be implemented right away - before we even need to tackle the core problem of cultural change.

Majority of cases involving avoidable death of a loved one, families pursue litigation route in the hope of truth, accountability and justice. Compensation (there could never be a commensurate amount for the avoidable death of a 22 yr old) not prime motivation
