I think the root cause is the pseudo court nature of the inquest. No "cross examination" allowed because it's "inquisitorial" but w/o cross examination it's hard to prove lies The "expert witnesses" who are called and trusted by the Coroner would be defendents in any other court

Very applicable to NHS independent review too. It is false to claim that these issues are not adversarial. The NHS clearly sees cases as adversarial but families are lulled in a false sense of security they are not and we are all working togther.

Hi John, I completely agree. I see a way forward through creating a framework where a Hospitals reputation is based upon it openly and transparently providing the best environment for patient care.

That might be a start, Rob. Has some merit as a concept. Although that transparency would have to translate into full and candid disclosure when care goes wrong too, for me to find comfort. I think our single biggest problem is medical record custody allowing for cover ups, tbh.
