      @TheDA_UK are campaigning for a just culture in the NHS. Avoidable deaths are tragic but may be caused by a multitude of reasons including system failures. A just culture seeks to recognise that rather than prosecuting individuals with no accountability for managers and systems

      They seemed confused about gross negligence manslaughter being a serious criminal offence too. Feels not so much about 'just culture', as 'just leave us alone' culture sometimes.
