There has been a tendency to blame the individual, says Prof Williams. Quick question to NHS complainants - did an individual get blamed when you complained? Or did they blame nobody (except perhaps you) and cover it all up? I don't recognise this so-called 'blame culture'.

Sorry is not in their DNA. I worked in senior level and when a consultant leans forward at a multidisciplinary team meeting as says, 'in my opinion' .... meeting over and that's it! ... try revisiting or challenging that and see what it does for your career!

Anyone placing themselves in a position beyond challenge, through whatever means, is likely as dangerous as they are delusional, imho. Such people need to be unceremoniously dethroned if patient safety is ever to become a real concept.
