There has been a tendency to blame the individual, says Prof Williams. Quick question to NHS complainants - did an individual get blamed when you complained? Or did they blame nobody (except perhaps you) and cover it all up? I don't recognise this so-called 'blame culture'.

Obviously I've not seen your submission, but I doubt cover up got a mention in the way you'd hope. Most I recall (though I was distracted a few times) was cover up mentioned as a 'natural' response to the prosecuting of doctors, implying prosecutions = cover ups.

My thoughts exactly. Gross negligence still needs to be punished imho, like anyone else in society where it leads to a death. But genuine mistakes are not gross negligence in isolation.
