There has been a tendency to blame the individual, says Prof Williams. Quick question to NHS complainants - did an individual get blamed when you complained? Or did they blame nobody (except perhaps you) and cover it all up? I don't recognise this so-called 'blame culture'.

Sadly John, both issues are true and they are both interrelated. Staff do often perceive a blame culture (at all levels of an organisation & the wider system) & the response to a patient safety event is often defensive, closed & dishonest. 1/2

We need a just culture that ensures individual staff aren’t punished 4 human error in a complex system, with greater accountability 3 managers & leaders 2 create & nurture the conditions needed 2 support safe care- where there is zero tolerance for dishonestly / covering up. 2/2

I obviously agree on zero tolerance for dishonesty/covering up, but examples of individuals being blamed are thin on the ground among NHS complainants, it seems? Who got blamed in your case for example, before you personally undertook to expose the truth? Anyone?

I have a letter saying drs didn't act on test results + withheld them from next hospital

Kid put in Care after tests + put on CAMHS unit. Nobody to blame is ridiculous.
