Success: Medicines and Healthcare products Regulatory Agency to review all Package Information Leaflets on Enalapril Maleate following investigation by Dr Helen Bright in the case of Dr Bawa-Garba

Investigation was regarding the death of Jack Adcock. Parliament has published our report for tomorrows session on patient safety: http://data.parliament.uk/writtenevidence/committeeevidence.svc/evidencedocument/health-and-social-care-committee/patient-safety-and-gross-negligence-manslaughter-in-healthcare/written/91579.html …

Thank you for that. An amazing document - I'm surprised you published it. Can you more simply state how the argument about religious uniforms applies in
#bawagarba? Thank you -
Sure. For decades uniforms have not been worn in mental health settings because of the issues of trust and rapport (medical, nursing, social workers used all to wear their uniforms before). Religious uniforms are off-putting for most people. No shame in honesty.

From what I’ve heard, there is a lot that isn’t in those documents...remember to ask yourself regularly ‘what am I not seeing? What isn’t here?’
-
Unclear which documents. Stuff not seen: What hospital training had Dr
#bawagarba done before her 5y paediatrics? She returns from maternity leave: what was her job with that hospital before? How long did the remediation take? Opinions of supervisors eg in the 5th year? -
What did induction consist of?( not a lot from what I’ve heard) What acute medicine experience did she have to that point, at what level, how recent? If the definition of gross negligence includes expectations of that level of training and experience, how was she assessed?
-
What responsibility does the doctor take in making sure these things happen before treating a patient? Would you get in a car without any lessons? Fly a plane? Drive a bus?
-
Dr themselves may not have much power to ensure appropriate induction and preparation for an environment they may not have encountered before. Something that is now deemed as standard expectation because of recurring issues in past and recognition no place has same procedures.
-
Responsibility of HR and department management to ensure staff fit for duties expected to perform.
-
Can’t speak for other specialities, can only ask ???, but GP & Pathol. training certain competencies assessed, and graduated responsibilities as deemed appropriate by trainer/supervisor. And can expect some scrutiny of skills and capabilities + supervision with every new post.
-
With that in mind training in most specialities has tightened in recognition of historical ‘get on with it’ attitudes that placed both patients and trainees at risk.
