But so many of the public want 'the crap'..... they want extended GP hours they want antibiotics they want to go to A&E for a cut finger they want a pill to solve life's problem AND we want good quality care when we and our loved ones are really ill and in need of care.

But so often we Doctors make the decisions ‘You will have these blood tests, these X rays and scans’ with no or little explanation

Doctors hold so much power in the Patient Doctor Consultation that we greatly influence the patient’s decisions
@trishgreenhalgh@JohnLauner -
#doctors are powerful so we must never misuse that power and use it always for the#patients benefit and not our benefit. Doctor and leader is simply serving humanity with sincerity honesty and integrity@willcpowell@GarethR08754256@didoharding@RobBehrens1884@RameshMehta15 -
Put an 85y patient with end stage renal failure before 2 renal Physicians, one will persuade patient to have dialysis and the other persuade patient to not have dialysis such is the power of the Doctor We should at least know the power of our position
-
And then be mindful of that power when it comes to a patient's consent being voluntary? Perhaps this list should explicitly include doctors, rather than just implying it with 'others'? https://www.gmc-uk.org/ethical-guidance/ethical-guidance-for-doctors/consent/part-2-making-decisions-about-investigations-and-treatment#paragraph-41 …pic.twitter.com/3DAlIP0S6h

Absolutely right. That's why the power of MH 'professionals' terrifies me. I only have the luxury of taking my current stance as long as I have capacity. WHOLE other game emerges once you cross over that line, I accept.

It's my decision alone. But the ugly problem which rears its head in the
#NHS is where arrogance & egos reign - "We know best". This is very dangerous for patients, their basic rights & indeed#PatientPatient as a whole is put at risk. -
Why does the medical profession lend itself to arrogance and rampant egos? This is not an NHS/UK phenomenon. It characterizes health-care in the Netherlands and it used to in Ireland too (improving now). If we could find what's causing it, we might be able to eradicate it?
-
We select brightest people of the society to be doctors. We dont select people for their attitude. Bright people also have strong personalities and most do not get feedback and they work in stressful environment. Most doctors are good and manage well but many need help support
-
I disagree. Medical Students are selected for being bright and having attitudes appropriate for a caring profession. Sadly the current
#NHS Money, Targets, Bean Counting and Bureaucratic Culture grinds that out of many of them within the Foundation Year@nlafferty@rakeshspatel -
What's the evidence that this is always the case Gordon? Years ago, St Georges Medical school had a notorious algorithm that weeded out foreigners & wasn't too keen on female applicants How much empirical evidence do we have that a more enlightened medical culture has spread?pic.twitter.com/nCQlHmS3e1

Couldn't agree more! My doctors are my expert advisors - the decision I make is mine alone (with husband). It might only be semantics, but "shared decision-making" does have a cachet of doctors involving patient in the decision they're making. This is the wrong emphasis.

If we Doctors learned to say ‘My recommendation is ... ‘ or ‘These are the feasible options ...’ that would place the decisions more into the patient’s lap

Views of doctors just that, views & can be subjective or biased by their own beliefs. In some health economies, independent 2nd opinion services are common & even required. It's little surprise these services are eyeing up the UK market given the 1 sided nature of the

Shared decision making may be the wrong phrase but an individual’s decision affects many stakeholders and they also need to be aware of that.
