As a final year medical student in 1986, I flushed a drip with potassium rather than saline. It was 3am, I was alone, I was untrained, I wasn't legally covered as a locum, the 2 ampoules were identical but it was my error & I was responsible. But in those days we hid our mistakes

"In those days?" That would be to imply a historical quality to such events, Phil. Covering up medical errors has become more prevalent than ever, notwithstanding any duty of candour requirements. But you already knew that. I've read your Medicine Balls column.
