I would have liked some evidence to use. The documentation surrounding mynGrandaughters care was not there. What should have been written down now never happend. The doctrin if its not written down it didnt happen and of no use whatsoever when you get to court. SAVED BY THE PEN

Sadly I hear that mantra over & over. 'If its not written down it didn't happen'. It's time we were believed as it's soul destroying, to not be.

In some cases it is still written down when it didnt happen! How many of you tweople have experienced falsification/fabrication of medical notes, documents. At meeting with GMC last December even their rep agreed with me that it was a *depressingly familiar* problem, & illegal!.

As to consent its often left up to the SHO, who in August and February time may not know his coccyx from his Olecranon and is just repeating rote fashion what he is told to say with very little understanding. After all theses Drs are not safe writing up drug charts unsupervised.
