That said, the easiest letters to write are the ones where you actually have a letter with some substance to argue with. The difficult letters are the slippery ones that don't actually say anything directly, replying to those is like juggling soot.

I would have liked some evidence to use. The documentation surrounding mynGrandaughters care was not there. What should have been written down now never happend. The doctrin if its not written down it didnt happen and of no use whatsoever when you get to court. SAVED BY THE PEN

In some cases it is still written down when it didnt happen! How many of you tweople have experienced falsification/fabrication of medical notes, documents. At meeting with GMC last December even their rep agreed with me that it was a *depressingly familiar* problem, & illegal!.

Intresting, working in procurement it is quite a simple task to reprint forms. If there are changes to be implement it only takes a week to get ok from relevant managers. For this not form not redsigned is quite frankly shocking..

This was always easy to fix. Only a lack of will has prevented the door from being slammed shut. I honestly believe this is used to cover up negligence at scale, which is why they don't want to put a stop to it. The truth would be far too damaging w/o this universal cover up tool
