Interesting stuff. I wrote a time management training course once which would've shared many themes with that. This phrase caught my eye re our letter exchange mind you: "If we think that we've got all the info when we've only got 7 bits out of 10, we can't go right"

I asked participants to remember these 10 items for 15 minutes - as you can see there were only 7. This illustrates how missing information means you cannot succeed at a task eg Diagnosing a patient’s illnesspic.twitter.com/7lmxMnu8eR

Then no one could recall all 7 items 15 minutes later yet most inpatients have 8 Diagnoses or Problems & often 15 minutes to 3 hours between pre-discussing a patient & seeing her on
#WardRounds - where we often test patient’s short term memory!

Appreciated. It's a valid point when relying on memory. But where facts can be repeatedly reviewed, memory is not a concern.
"Missing information means you cannot succeed at a task"
Now change 10 to 21 - how many are missing? And why doesn't it also matter re #BawaGarba's errors?
