The NHS does get things wrong, as do individual clinicians, and it’s both regrettable and terrible when these mistakes occur, for the individuals concerned and their families.

No, it’s not. There are cases like Shipman and so on, but they are thankfully rare. I have worked with Drs and also been involved with SUI investigations, and believe me, there is no opportunity to cover up as you suggest
