      How steady was the environment you worked in? Were you subject to large variations in volume or complexity of tasks, or access to supporting services (IT and human), or wild variations in types of tasks required of each of you?

      Healthcare probably isn’t the only such industry with such variability and strains, but it is one of the most heavily scrutinised and criticised, just because of the fact that it affects virtually everyone at some time in their life, and we all pay for it in our taxes

      Healthcare is the one industry that you can't easily walk away from and choose not to engage. It's not a choice. But I still think any exceptionalism arguments should be avoided at all costs. History teaches us that such thinking rarely ends well for the rest of society.

      It’s not exceptional it’s, its realist. And I agree it should not be that way. For now we are stuck with these realities, and in many ways getting worse. Personally, I think continuing efforts to try to ‘improve’ but using same limited methods is adding to problems.

      With respect, you started out suggesting the need to document everything accurately was exceptional. When I pointed out it wasn't, you've looked to try & identify another distinction instead. I don't doubt your world is a challenging one, but we expect health records to be right.

      I think you mistake me, John. I agree, accurate, contemporary notes are extremely important. My point was that the ‘ideal’ and the ‘real do not always match, for a multitude of reasons...

      I’m not excusing it, but acknowledging that the ideal ‘work-as-imagined’ is faced with challenges+++ in the messy reality of real life as a Dr/nurse, made all the worse with increasing, unchecked complexity and workload.

      I must say, I think @Jeremy_Hunt ducking the widespread calls from all sides to set #safestaffing levels is a legacy that will haunt him no matter where he goes in life, or how many trees he hides behind.

      You can add individual statutory duty of candour to that list for me. Another thing which Hunt fudged during his tenure to ensure it didn't happen. Without true candour, no other measures work. Crap in, crap out and all that... #RobbiesLaw

          You have a statutory individual duty of candour for HCPs in Scotland? Not just a professional one? Really? Don't suppose you can point me towards the statute which makes that clear, could you please? Very interesting. And don't start me on #Gosport. You'd be here a while! :)

          No Scotland doesn't have a statutory individual duty of candour. It is an organisational duty which has no enforcement criteria as Scotland (unbelievably) has no independent health regulator.

          Oh! So Cathy was incorrect about that then? Perhaps explains the lack of response? And you don't even have an independent health regulator? (Not that ours South of the border is worth a damn). But how does that work?? Throw in the strange stance on 'intent' & it's very confusing.

          It doesn't work at all especially for families let down by sub standard care. We only have an NHS in house quality assurance body (quango) which regurgitates IHI methodology and has no surveillance or intervention powers and cannot investigate individual cases.

          Oh dear God! And there was me thinking we were holding the shitty end? Never trust a politician who starts out trying to convince you they're being clear. 'Recognised the quality of Scottish healthcare'? Easy I guess, if all you do is add up the 10/10s they give to themselves.
