      Is that ‘failure’ deliberate, to cause harm? Or in itself due to inexperience and limited knowledge? The Dunning-Kruger effect, along with lack of awareness of importance of features or effects. That’s exactly why juniors must be appropriately supervised

      So is junior Dr, in unfamiliar environment, forced into unsupported decisions at limits (or beyond) their competency level because they are left to get on with it- have they made a conscious decision to take shortcuts knowing that would harm? That’s definition of Gross Negligence

      Then every junior is guilty of gross negligence, purely by nature of lack of knowledge and experience? The more you learn the more you realise you don’t know. That is purpose of ‘training’. The grossly negligent trainee is one who purposely rejects their learning needs

      I see elsewhere that as an ST6, she was close to being a consultant. So just how inexperienced can we really consider her to be? Can't imagine she'd be convicted of GNM for not doing something she knew nothing about. Rather for failing to do something she ought to have known. No?

      Paediatrics is an 8 year training pathway btw. So not weeks/months off being a consultant

      Fair enough. I'd still say I must assume that whatever got her convicted was likely to be a failure to do something she should reasonably have been expected to have known though. I'd find it inconceivable that it could be successfully argued in court to be a failure otherwise.

          Unfortunately I cannot share that confidence in the English justice system. The reality is that the jury’s opinion was of something truly bad, and many of the errors are based on expected normal management and procedures, but based on consistent environments, information and team

          We see it time & time again where experts create ‘clear’ guidelines/policies to follow in particular conditions, but can rarely be followed due to variable other factors not accounted for by the experts, who often work in different specialities/environments/patient groups

          Easy for ‘expert’ to state should do this or that, must always do... But very often is based on a clinician dealing with single patient in 1 place, with all IT working, knowing exactly how that hospital works, with adequate numbers competent trained nursing & other medical staff

          Try passing driving test in quiet town, not driving for year, and then hiring car to drive in Paris. How good would your decision-making work then? Then add broken Sat-nav, & a brand new tour guide just pulled in from Rome to cover who only speaks Italian... will it go smoothly?

          Sounds totally unrealistic, yet the cognitive strains and risks would be close to what has been described for BG, and still happens elsewhere. She was just unlucky that something terrible happened, whereas likely same for others, who just got lucky and nothing major happened
