Often forgotten but in my opinion, most important factors in pt safety is timely assessment, robust and competent assessment. This needs critical thinking and clinical medicine is a losing art, increasingly replaced by investigations. Not good for pt safety.

Agreed, to a point. But when that 'timely, robust and competent assessment' is absent, as it was in the #BawaGarba case, then such investigations are required for pt safety. It's a lack of one that provokes the other.
