I just think that blaming a single person under the circumstances that occurred that day doesn't really tell the whole picture and leaves the door wide open for the same thing to happen and no learning to happen.

@MartinBromiley,@UKSepsisTrust - better for patients after devastating consequences for individuals & families. Not saying there weren't things done poorly by BG, but all previous reports say she was a conscientious doctor - doesn't that raise questions about circumstances? -
