I started as a Doctor in 1980 and I’m not sure I have ever seen a fluid balance chart in an acutely ill patient that I have believed. At least in the 1980s staff tried to add them up but these days it’s usually very murky and hard to work out what went in when and what came out

I think it depends on the context, the nurse/ staff ratio in CAU that day was ok. They were concerned about his fluid balance due to the cardiac issues, that should have made accurate documentation even more a priority.

So why not in those circumstances get PICU or the on call Consultant involved? Being a patient brings a totally different perspective.

I am told on call Consultant was 10s of miles away. That was an Organisational Failing and way beyond

Why was her consultant not there? In the *same* hospital, there would be 2 consultants looking after acutely ill *adult* patients from 8am to 5pm on site, all other commitments cancelled, 7 days per week.

The vast majority of Consultant Paediatricians I’ve met are kind lovely people who are obsessive about attending their patients and supporting their Juniors with their presence and expertise. Jack nor Dr

In my current job we are just 4 Physicians and when on take the on take Consultant is always, always in the Hospital 9 to 6pm & we know our Juniors are vulnerable at night - they even have to assess sick babies - we may get care wrong but we strive to at least be present

Imagine, if all the circs surrounding unnecessary deaths since Jack and all that will happen in the future get same level of scrutiny immediately and not 7 years down the line..might be able to stop the unnecessary deaths then.

I believe that there have been significant changes to the unit following this case and not just because of the media furore.

Funny, but that's not the impression I've been getting lately. I've seen indication that the problems persist in that hospital, despite the media furore.
