You made similar comment before. Has nothing at all been changed in response to this case which will help prevent a repeat then? It's hard to conceive that the inaction could persist beyond the court case.

It makes raising concerns an ass covering exercise in case things go wrong, when all you want is for it to be addressed. Unacceptable.

And that tool is painful to go through. It's like a constant escalation process that almost seems to anticipate you won't be satisfied with the response to your concerns. Perhaps a member of the public should report the trust to the police? Does their inaction not endanger life?
