      Yes. I'm still at a point where I'm the walking wounded, a victim of appalling treatment by an NHS trust, before and after my brother was very efficiently left to get ill and die under their hands. That is the whole truth. It's not the only trust. The system needs a clear out.

      Not sure any of us can be smug, regardless of our valiant battles. We are all battlers, in tough lives. No judgement but genuine questioning. Why do medical leaders not usually practice committed, just leadership?

      Dear John I respect you a lot. Never Event is something I do not like the term. #NHS expects Never to happen! Sadly it happens and some panic and cover it up! When I investigated revealed amazing findings and learnt a lot. In 2 months there were 73 Never Events in #NHS.

      Thank you Umesh. In this instance, my reference was firmly tongue-in-cheek, but it's a subject I do take very seriously, as my mum was a victim of wrong site surgery - still no.1 on the list of Never Events and always has been, since the original list of 7 was created years ago.

          Thanks John. Any preventable death is a tragedy and we doctors and leaders must prevent them. Having given opinion on 22 children who have died and not even on should have died I am determined to make sure our #NHS becomes safest the best. Many of my replies are tongue in cheek
