Good point. Nothing personal but leadership is listening and responding on what you might not want to hear and learning from it. A quality wholly lacking in the entrenched mind set of far too many CEOs and senior management of NHS trusts.

Thank you Umesh. In this instance, my reference was firmly tongue-in-cheek, but it's a subject I do take very seriously, as my mum was a victim of wrong site surgery - still no.1 on the list of Never Events and always has been, since the original list of 7 was created years ago.

Thanks John. Any preventable death is a tragedy and we doctors and leaders must prevent them. Having given opinion on 22 children who have died and not even on should have died I am determined to make sure our
