There may have been a problem with the syringe drivers and of course needs looked into if there is concern. But having read the report the practices on the ward were really very abnormal and this just seems to muddy the waters to start talking about the drivers

The big problem with syringe drivers: Type 1 - 24 hrs morphine dispensed in 1 hr Type 2 - 24 hrs morphine dispensed over 24 hrs Both look similar, so easy to line up wrong driver on patient & kill them This is where the inquiry must focus

This is where the inquiry must focus? I don't think so. It doesn't take 650 dead patients before you work out you're using the wrong driver. And it doesn't explain why so many of the nursing staff were raising these concern - in 1991.pic.twitter.com/8GGOrP5hCR

Far from it. Drivers will get their due consideration, but only in context. I for one won't allow drivers to be turned into Nelson's eyepatch though. There's already enough wilful blindness on display from NHS in their cover ups. But 'I see no murders' is a step too far for pts.
