Of course views all mine. All without prejudice. Just a regular chap after all. Oh...and RT's may equally imply ridicule as endorsement.

"Gosport inquiry 'ignored' evidence about syringes" > The dyke is crumbling around Jeremy Hunt's finger and the cover-ups are threatening to pour out. But don't fall for the spin here - syringe drivers didn't kill a single patient. Those who used them did.http://news.sky.com/story/gosport-inquiry-ignored-evidence-about-dangerous-syringes-11415289 …

They must not blame the syringes. If one malfunctions it will. be removed from service. Each has a specific number.

First of all they need to make up their minds - did the syringe drivers malfunction, or was there a 1hr version and a 24hr version that were being mixed up with fatal consequences? Always a good idea to get your excuses clear in your head before spouting them!

The syringe driver is checked every hour by staff. 2 nurses will have set it up including speed of admin as controlled drug

It was commonplace for staff to leave patients and families alone for very long periods of time to have time alone with LCP.. those 4 hour checks often didn’t happen for this reason. Good intentions...

It would have to be reported as a drugs error if the wrong amount had gone through regardless

Jees! It seems so basic as to be something you'd assume would never be allowed to happen. Especially when the risk is so great.

Much harder to spot that error though than one simple syringe driver leading to one person

I do think they're trying to dilute the blame. Very few drs have ever been accused of killing

Considerably more nurses have been convicted. That may be who gets blamed yet.

It's a hierarchy of protection. Throw the lowest common denominator to the wolves to save yourself. When you can't contain the scandal to nursing staff, throw them a junior doctor - like they did with Bawa Garba. Consultant goes unscathed.
