They must not blame the syringes. If one malfunctions it will. be removed from service. Each has a specific number.

First of all they need to make up their minds - did the syringe drivers malfunction, or was there a 1hr version and a 24hr version that were being mixed up with fatal consequences? Always a good idea to get your excuses clear in your head before spouting them!

The syringe driver is checked every hour by staff. 2 nurses will have set it up including speed of admin as controlled drug
