"Gosport inquiry 'ignored' evidence about syringes" > The dyke is crumbling around Jeremy Hunt's finger and the cover-ups are threatening to pour out. But don't fall for the spin here - syringe drivers didn't kill a single patient. Those who used them did.http://news.sky.com/story/gosport-inquiry-ignored-evidence-about-dangerous-syringes-11415289 …

Depends on the death certification process.
-
It does? Maybe overall, but I wouldn't expect to have to look past the local staff on the ward for alarm bells. Only takes 1 experienced nurse to figure out patients are dying when they shouldn't be. Gosport had 7 of them after all - raising 10 points of concern early on.
-
I haven’t read the report.. I’m not surprised though. When nurses fail to be heard you need a robust backstop and I think independent medical examiners would provide that.
-
I totally agree. And that backstop ought to identify those engaging in cover ups too. I've not read much of the report either yet, but that detail was on pg 4. Detailing a meeting where the concerns were formally raised in July 1991! Those who suppressed it are equally guilt imho

1991.. it’s unreal isn’t it? (Unless you’re a family membe of someone who died when it’s all too real)
