      I guess if staff thought they were giving the drugs over 24hrs and weren’t aware that the syringe drivers were different then the staff didn’t know they were doing something wrong. But if they found the syringe driver empty after a couple of hours, they should have reported it.

      That's true Jim. Of course, the suggestion here is that if they'd made that mistake, they may not just find the syringe driver empty after 2 hours - they may also find a dead/dying patient attached to it too. How many does it take before it's figured out? Not 650, is all I know.

          It does? Maybe overall, but I wouldn't expect to have to look past the local staff on the ward for alarm bells. Only takes 1 experienced nurse to figure out patients are dying when they shouldn't be. Gosport had 7 of them after all - raising 10 points of concern early on.

          I haven’t read the report.. I’m not surprised though. When nurses fail to be heard you need a robust backstop and I think independent medical examiners would provide that.

          I totally agree. And that backstop ought to identify those engaging in cover ups too. I've not read much of the report either yet, but that detail was on pg 4. Detailing a meeting where the concerns were formally raised in July 1991! Those who suppressed it are equally guilt imho
