Pɾҽɱ, thanks for asking. The fundamental reasons are usually always the same. A lack of vision and leadership. The technology always, always requires vision. It can not come from the engineers and not from the CEOs (usually). Also regulation barred Bell Labs from commercializing.

Pɾҽɱ, Great question. This is the challenge of every tech company today. It is why there is a waning and a wanting from consumers. There must be an evangelist and advocate that faces the company and the consumer. Steve Jobs did that precisely, he did not have to be a CEO.
