I can't think of another region where it works like this, does it? New England is never used in a way that excludes Boston, the South doesn't normally omit Raleigh, Atlanta, Richmond, etc., but the Midwest is always construed as a region of small towns and farm counties
I think that comes down to the degree to which 'the Midwest' has been mobilized for so long as a 'real America' against which the coasts are set.
