Tolkien's sketch of the hornburg as he imagined it has more projecting towers (I count five on the deeping wall) , with the keep projected forward to force enemies to move around the hornburg to attack the wall.

The lack of zig-zag placement on the gate is a design choice, rather than a flaw and it actually makes a fair bit of sense if the goal is to stage lightning cavalry raids out of the Hornburg. Roman forts do not offset gates, for instance. They seemed to work fairly well!
