I don't know what you're seeing at vaccination sites, but the CVS on Winchester road northeast of Huntsville today was doing a well-organized, maximum capacity (distanced), continuous operation midday today. I can't imagine any appointment slots were going unused.
I got my first shot on Tuesday at a Walgreens and they were humming right along moving people through. They had a dedicated window for check-in. I do wonder what the follow-ups for second shot will look like though. Hard to remember, people forget, etc. etc.
