We also don't talk enough about the privilege built into the process of choosing programs. For every disadvantaged applicant getting great advice from a Twitter thread there's three or four who were just prepared far better by wealthy undergrad institutions or academic parents.

Right - and many of the programs being called "bad" in that framing instead of "collegial" are in fact extremely proud of their "competitiveness" and "rigor." These cultures aren't emerging by accident.
