Yet we need to understand it! How could such a decision be made, w/o understanding the threat it posed, not just to SQ, but to the inmates being transported there? One person not understanding, is one thing. But an entire decision process from conception to execution is another!

I think in addition to the measures you outline, the lessons need to be institutionalized, with infection control considerations being built into transfers. Influenza comes every year. We shouldn't wait for an epidemic to go find an epidemiologist!
