This was something I argued to change when I ran for Congress. The US doesn’t have enough doctors and it is partly because we choose not to train enough. “Chose not to train enough” is an interesting way to put it, I think you mean “young people are choosing not to agree to self finance a quarter million dollar education so they can serve the public good”.
I would say both are true. In addition to the high out of pocket costs, there are also limits on the number of slots at med schools as well as limited residency slots. All of the above need to to be fixed.

Agreed. My friend has a smart, caring son interested in science and medicine, but is scared of the burden of the cost. They are upper middle class. Are our best & brightest able to take this on, or only the uber wealthy or highly debt risk tolerant? Is that the best talent pool?

Exactly. We should be trying to get the best people for the job, not just the ones who can afford it.
