I'm a teacher. I have insurance. But, emergency room visit starts at $350. Ambulance starts at $1000. Instead, I crawled (I literally couldn't stand up from the pain) to my kitchen, reached for my prescription ibuprofen, took 2 of them, & crawled back into bed with a heating pad.
My wife is a teacher too and, while she has never experienced what you had to, I know that she waits as long as she can before seeing a doctor, because of the cost.

I pay for our highest-priced insurance plan in exchange for lower copays for HMO docs and the ability to see a doctor on the "Tier 2" list without a referral if I need to (but at 30 % coinsurance, of course). It's still not great.

Ambulance @ $1000 ! You want to ride in it, not make a down payment. In Ontario, $45 Cdn charged to patient (and that is covered by extended health care) Cost to patient $0.
