A person undergoing an existential crisis is feeling ... existential perplexity? Existential distress? "Spiritual dryness" could also work, if you're o.k. with extra connotations of sadness, loneliness, loss.

"purpose anxiety" is a fine term. I opine that higher order uncertainty of this kind is always the fundamental cause of anxiety.
