In situations where traditional etiquette requires a nonsensical "sorry" (someone bumps me while walking) I strive to substitute a so-stern-as-to-be-clearly-comical "HEY! Don't let that happen again."

Yes, yes! That response only reserved for someone who is already apologizing profusely to me...when I do it, nod/smile works great.

The Midwestern “sorry” is apparently more capacious? We use it to mean “I feel bad that you feel bad,” not just the more specific “I feel bad that my selfish decision has made you feel bad.” Kind of wonder if confusing situations like your door thing led to that mission creep.
