Trans people live lives on the margins, which means treatment with a particular doctor/clinic might be interrupted for financial or geographical reasons. I've never voluntarily stopped HRT. I did once go about a month between shots, bc I couldn't get an appointment in time.
As a rule too, I don't think trans people would want there to be some central record to keep track of these things across states. Being able to be unnoticed is rather important
