    Averil was failed by every organisation. The report took four years to complete and even after it was published, services have not improved. "Short-term reputational management" takes precedence over open and honest learning.

    Something to learn from. All bodies need to utilise eating disorder treatment guidelines thoroughly and make sure these guidelines are written with all evidence/case history available.

    Article yesterday written by Baroness Parminter on where eating disorder services vitally need to change, using Averil's story as a prime example of why the changes are necessary. Parminter will lead a debate in the House of Lords today on eating disorder services.

    Dr. Chess Denman emailed CPFT clinicians after Averil died to say, “I have concocted a plan”. Since then ongoing cover up by the Trust and the board. RIP Averil we miss you x

    Sensible comment by on If the NHS investigated properly, recognised it had harmed patients and offerred compensation rather than deny and defend it would save almost all of the legal costs. Even better to prevent the harm by improving patient safety

    Such a cruel, evil and very dangerous illness that consistently appears to be misunderstood, in our experience you need professionals who know what they are doing, are highly skilled & focused, this has not been our D or our experience, words fail, so so sorry

    It seems services r v gd at talking the talk, but not walking the walk-making bold claims & being given titles, yet speaking 2 pts the story is v different. My best friend took her life on the UKs leading National ED unit, whilst they spent 18m trying 2 discharge as “too complex”

    Dr Serfontein, Consultant Psychiatrist , calls for more ED psychiatrists to reduce risk in community treatment. This coming from the same ED psychiatrist who was allocated to care for Averil but failed to see her until she was dying.... RIP Averil

    Harrowing story of a young man with autism who should not have died. Hospital dismissed death as natural causes, no post mortem, no inquest. Such an indictment of the way these deaths are investigated. A family forced to fight for accountability.
