      The key lessons are unchanged: The seriously ill have goals for their care besides just survival. When we don't ask what they are (what tradeoffs they'd make & not make; what quality of life is unacceptable) and tune care accordingly, incl new treatments, the result is suffering.

          indeed, it seems the standard of care must continue to shift from a doctor-centric perspective to a patient-centric one, which prioritizes patients' unique needs. I, too, believe meaningful care is much more than treating disease according to protocol.

          Would challenge that this is the same values calculus those whose lived experience suggest benefits in regard to opioids and risk willingness for intractable pain. Opioids no more effective in cancer— suggests little difference for severe djd of spine etc?
