I’m living this article right next now. A large health system merger causing the need for major EHR transitions. I feel like each & every day in clinic i’m walking in a patient safety minefield trying to avoid med errors & quality deficits. It’s exhausting and no end in sight.

Great article,I would add from Physician standpoint disruption of established collegial reference networks is a big disruption to patient care and Clinician satisfaction.

One common argument is that expansion can enhance quality because acquiring hosps often have more advanced quality programs than acquired (often community) hosps and because larger scales enables more robust analysis of quality data (greater n). Did you see any evidence of that?

Although we did not look at that in this work I have certainly observed the difference between the "haves" and "have nots" hospitals. Have seen latter look forward to affiliation specifically to get resources to measure/manage quality.

The story in India is even more glaring! A systematic explosion of the private sector in Health accompanied by diminishing Government spending is costing rural sector, which is close to 70% of India. Wonder, what would happen if private sector mergers and expansion occurs??!!

This is excellent review of merger challenges. Financial integration often overshadows clinical integration. Hope that proposed BIDMC-Lahey merger understands these issues.
