Nearly half of births are covered by Medicaid, and the rates paid to hospitals and physicians for prenatal care and delivery are too low. I would guess, lower than costs. We love Medicaid expansion, but how about increasing the rates!

And if anyone wonders why OBGYN doctors (and Peds) in rural areas are employed by hospitals for higher "provider based" rates, look no further than Medicaid rates.

The elephant in the room is the med malpractice climate here that forces every obstetrician to practice defensive medicine- resulting in the highest c-section rates in the world. Bad for mothers- but good to keep trial lawyers at bay.

I think it has more to do with our overly invasive and technology based way of doing birth. More women are giving birth at home for this reason.

This may be partly business, but also the fact that medicine cannot predict which pregnancy will require expensive technical help and which will be the most natural process of health man experience. So we medicalize everything. It becomes the standard of care.
