Indeed amazing progress, still a long way to go for many cancers. @BME_CANCER @abcdiagnosis @Mike_Thorpe_ @louisebrady17 @UzmaMD @shaheedashaikh @MissLivJoseph @Catz75Cat @liz_leaver @Liz_ORiordan @trishgreenhalgh

Cancer is complicated - helpful to distinguish broad groups based on site &stage ( most live 10, most die 1 year, & a middle chronic group
-
How much does that translate into living longer rather than just being diagnosed earlier?
-
This question has been asked by many to original
@Atul_Gawande tweet. Early diagnosis has many other benefits. Less than 1yr for most one extreme does need more work! -
No doubt early detection can mean easier to treat but we do need data on this. Prostate cancer can be over diagnosed so it is very complicated Anil.
-
Screening.. (benefits & risks) needs to be clearly explained in primary care, with space & time for discussion.. ..key role for all 2 play!
-
Spot on Louise. We also can spend a lot of time in primary care during aftercare, more responsibility there with supported self management.

-
You spend a lot of time in secondary care (if you are lucky enough!) but be careful with supported self management especially with BC...

How much of this is lead time bias? 5 year survival changes are difficult to interpret
-
Yes! I worry about lead time bias and overdiagnosis. I’d love to see adjusted mortality data instead as an indicator of ? progress.

Interesting how the impression one gets from the data varies with how it is presented. I'd say 5M pts in clintrials -> 15% increase in 5 y survival a little disappointing.pic.twitter.com/b3p95nqif3

Some real progress. Some eg
#prostate cancer also related to lead time bias, over-treatment etc. While no improvement in other common cancers like#bladder#cancer survival, perhaps more needs to be done....https://twitter.com/atul_gawande/status/928241621837197313 …Thanks. Lead time bias. Overdiagnosis. What’s the actual death rate change?

This is wonderful, but I wonder why the only cancers with regressions (cervix, uterus) affect women exclusively, and the greatest progress (in % gains) is in a very male cancer (prostate). And Movember is still "a thing."
