There are not enough midwives. Doctors lead birth in the USA and that is a design flaw. U.K. spends far less for better outcomes.

No, our federalism. I used to review Maternal Mortality for my state. There is a CDC program but no teeth. Single-Payer would help.

A fair amount of healthcare data comes from billing. Single payer means one set of data for all.
