Move all appointments (except urgents) to 30 minutes & accept a 5% pay cut. Most groups will accept this. Waiting for them to change its passive & not in their interest. Beat burnout by setting your own standard.

Many colleagues choosing
#directprimarycare for just this reason, taking back control of their patient care. 5% pay decrease untenable for many colleagues however. They already do so much that is uncompensated. Perhaps@Atul_Gawande can support#DPC -
DPC is great for the well off but who will care for the low income populations?

DPC, from I have seen, is more affordable for low income than many current health insurance premiums. DPC is distinct from concierge care which is geared to high income, two different things.

Absolutely...primary healthcare providers need to move away from being ‘gatekeepers’...A restructuring to a central and continuous role in the care of the individual and family at all times. That would need a healthcare overhaul in the US!

Knowledge is a barrier. Strategic decisions for common scenarios are complex, and nuance may only be accessible to experts. PCPs can’t be expert in all areas & spending more time with patient won’t help. Need better communication between specialist & PCP to help PCP participate.

I often wondered why health insurance separates dental and vision. Next thing you know, there will other separated insurances. Like one for you appendix, one for you lungs. Doesn’t make sense. Everything in the body is connected.
