Hospitals are, after all, just buildings. They do not admit patients. They do not diagnose, prescribe, or treat. CC:

"NPs grew 109% from 2010 to 2017, RNs grew 22% from 2.5 million to 3 million and the number of doctors grew 9%" which is why there's a shortage of docs across specialties... via

(2)/fin Many AMCs are part of an ecosystem of care that helps train docs and others for states/regions/nation. Example is where i trained at Univ of CA...here's 8 years of grads' locations

2 more points and i'll stop (1) Many states are net exporters of trainees...many factors on why people stay or not

Compare that with retention in California, where about 2/3 stay in state It's complicated.

Lots of people complain that "we just need to take away training spots from XXX state". It doesn't work the way they think. For Iowa, only about a third of GME trainees stay in state.

"in the past five years only 31 percent of family medicine residents in this program have remained in Cedar Rapids after graduation" I hate seeing programs close but this reinforces a critical point--practice location not just about where people train

One hospital tells Rau its “commitment [to thorough identification & reporting of infections] make our rates appear to be higher than some other hospitals.” Surveillance bias is a legit concern. We no longer use CMS HAC metrics in our rankings.

There are still issues with the methodology, surveillance bias and the inability to fully risk-adjust for patients who are sicker and who are more likely to have multiple comorbidities
