    Term used loosely describes radically different symptoms requiring different responses: 1)normal & - no treatment 2)milder depressions- watchful waiting/ 3)severe depressions are like "brain disease"/dont respond to therapy/do need med/ECT

    This is extremely misleading- no one size fits all. or watchful waiting is 1st line for milder/transient . But meds or ECT are needed for severe depression..

    People from other are routinely misdiagnosed for having voices/visual hallucinations that make perfect sense within their cultural context. Misdiagnosis also frequent in people with hallucinations due to deep over loss of departed loved ones.

    In my experience, psychiatric over-diagnosis is most common when the patient's life stresses & cultural differences are not taken into account. Clinicians should always beware of their own cultural biases & ignorance.

    Most "Awareness" weeks are a shabby sham invented by drug companies to peddle pills. But increasing awareness of & treatment for is crucial. Cause misery/dysfunction/illness/death. And many people are more ashamed discussing their eating than sexual problems.

    I think its absolutely crucial continue to grow in its indications & increase in its flexibility. Psychoanalysis got frozen in time because Freud's followers were far too worshipful. followers should be inspired by her but not scared to develop DBT even further

    . Not "innocent" but more clinically sophisticated time. You endorsed Goldfried & Davison's 1976 "Clinical Behavior Therapy" saying it could help one to be "a complete therapist" You & I wrote about transdiagnostic case formulation, mediators & moderators .

    There's pervasive media bias toward hyping treatment benefits while neglecting negative results & harms. Big part of this is reliance on experts with financial &/or intellectual conflicts of interest. Att'n journalists: Use this list 100 independent experts to correct imbalance

    Clin Psych Controversies I would add: 1)inventing new terms for old concepts 2)developing another school of therapy 3)whether research being done is relevant for practice 4)how to train therapists 5)whether ESTs work for everyone 6)confusing ESTs with EBP

    Cheery once again completely misses the point. He/his agency are pretty useless unless they face fact that 600,000 patients are jailed or homeless. Balance your research budget away from narrow brain/gene agenda. Do more to really ensure better care in real world.

    We wrote "Differential Therapeutics: The Art & Science of Treatment Selection" 40 yrs ago- an innocent age when psych evaluation was careful/treatment selection still mattered. Soon after, the aggressive/misleading marketing of new SSRIs reduced evals to "Blah/Blah/Blah/Prozac"

    CONSIDER THIS: Hospice patients not only die much more comfortably, they actually also live a bit longer. Palliative care beats heroic care on all dimensions: lifespan/humanity/cost/family closeness. Always ask for hospice consult before accepting invasive end of life treatment

    Modern medicine has sold the lie that we can cheat death. "Heroic" measures often shorten life/make its end lonely/miserable. Epictetus "I cannot escape death- but at least I can escape the fear of it." Seneca "The wise man will live as long as he ought- not as long as he can."

    In 19th century, death was almost always at home surrounded by loving family. In 21st century, death is almost always lonely- in impersonal hospitals surrounded by strangers/invaded by tubes. Technology replaced humanity. Hippocrates “Cure sometimes/treat often/comfort always"
