Why? Precisely because, and not in spite of, the fact that healthcare is a commodity. Healthcare makes no distinction between who is paying for it. The going rate for healthcare is determined by aggregate demand, not individual demand. Therefore transitivity applies.

Solutions: Either make healthcare not a commodity, something distributed purely according to status rather than ability to pay, or accept the implications, which render pseudo-market mechanisms for providing healthcare into a massive externality generator which has to be handled.
