An action is immoral if the expected outcome is suffering, even if you got lucky and everyone enjoyed it. Eg slipping LSD into an office coffee pot

I'm fairly certain most people have a poor understanding of moral epistemology and the differences between different deontological ethical and normative ethical systems and considerations.

That’s a hard question. What if u feel good about it first but later look back on it as negative? Not sure I know how to answer.

Depends on how far you extend 'all parties affected by the action'. Whether that extends into the future or just into the present moment.
