Although, the obvious case seems like if two people are both inebriated to the point that we consider sufficient for making consent impossible.

there's some Gettier-type situation where this should be possible...
like: person A intends to rape person B, asleep in a bed. Person C learns this, and gets into the bed pretending to be B

I would say no. I think when you're raping someone, you're necessarily giving them YOUR consent.

I'm confident that someone can come up with a crazy edge case in which two people mistakenly believe that they are raping each other, but I doubt you could get me to take it seriously.

The simplest way I can imagine is if both participants doing something to the other person that that person doesn't want at the same time
Similar to how if you don't wanna be tickled you can still tickle the other person while they're tickling you
