How can I help someone who's obviously distraught and sobbing, but also detached from reality, having a psychotic or schizophrenic episode? Sometimes I want to help but I don't know what to do or what they need.

yeah but if it's like, someone on the street? and they aren't currently in the mindset where they'd be receptive to strangers telling them to go to a doctor?

It depends on the severity and amount of danger you’re putting yourself in.
But, managed distance. Give them security by removing triggers, decreasing stressful stimuli, and offering direction, while staying at a distance. Distraction can work. Otherwise, crisis lines are great.

Sometimes just being there for quiet support and to be sure they’re safe is best.

A calm, reassuring presence can often be helpful. A low stimulation environment might also help.
If the person is in danger of harming themself or others, call 911 and they can go to an ED for evaluation and treatment.
